Please note that the $100 registration fee for
competitive swimming will be assessed upon receipt of this form.
Also, you must send in a copy of your Birth Certificate to: STAR
Aquatics, Attn. Doug Cornish, PO Box 16311, High Point, NC 27261.
Once this
Change Form is completed and submitted, the information will be sent
directly to STAR Administration. Any applicable fees will be billed
with the next cycle. |
